If the title is not decided, then they will show Manchester United or Chelsea
If relegation is not decided then they will show a relevent game from that.
Champions league places are already decided, so they'll possibly show a UEFA cup "play-off"

Looking at the fixtures the other game they will choose will more than likely be.....Sheffield United vs Wigan or Liverpool vs Charlton.

If Man U win the league (which its pretty much nailed on) then the West Ham game is when they will receive the trophy as it's their only remaining home game. Not necessarily, they will have cameras at all grounds so can just cut to OT for the presentation.
