All is not lost though, it costs nothing to change your name so nip down the registrary office tomorrow and change your name to match the one in the book.

Well, last year they didnt put my name in the book at all, as my order was included in the dvd orders. And absolutely no chance I sent it in wrong, I checked, checked and checked again and then got someone else to check it. Pity the same wasn't done during the production process.
Hopefully it hasn't happened to anyone else.
